PRIVACY NOTICE

We are committed to protecting your privacy and personal information.

Peninsula Hearing Services Inc. collects, uses, and discloses personal information in accordance with the Freedom of Information and Protection of Privacy Act (FOIPPA), Health Professions Act (HPA), and other applicable laws (including the CHCPBC Bylaws).

This Privacy Notice includes a summary of information about our practices for collecting, using, protecting, storing, and disclosing personal information. It is intended to inform you about our practices regarding the collection, use, and disclosure of personal information in our custody and under our control in accordance with the above-mentioned legislation as well as our internal policies and procedures.

WHAT IS PERSONAL INFORMATION?

Personal information is recorded information that can be used to directly or indirectly identify an individual. For example, this includes but is not limited to, an individual’s name, address, date of birth, hearing aids and physician's name.

WHAT IS NOT PERSONAL INFORMATION?

Personal information does not include business contact information. Business contact information is used to contact an individual at a place of business. This includes but is not limited to, an individual’s name, position title, business phone number, and business address, when used to contact a person at work.

WHY, WHEN AND HOW DO WE COLLECT YOUR PERSONAL INFORMATION?

We collect personal information to fulfill our goal of providing client-centred hearing care. We may also collect personal information when a patient allows or requires us to.

We generally collect personal information directly from the person the information is about, unless a patient allows us or requires us to collect the personal information from another source. When we collect personal information directly from you, we will tell you why we are collecting the personal information.

When you visit this site, the web server automatically collects a limited amount of information essential for the operation and security of our website. Some of this information (e.g. browser type) does not identify you, while other information, such as an IP address, may identify you. However, we do not attempt to link IP addresses with the identity of individuals visiting our website.

HOW DO WE USE, DISCLOSE AND STORE YOUR PERSONAL INFORMATION?

We only use and disclose personal information for the purpose for which the information was collected, or for a purpose requested by the patient on whom the information was collected. We only keep personal information for as long as reasonably necessary to fulfill those purposes, or to meet legal requirements. When we use personal information to make a decision about an individual, we keep the personal information for at least one year.

HOW DO WE PROTECT YOUR PERSONAL INFORMATION?

We take reasonable steps to protect personal information in our custody and under our control. This includes administrative security measures (e.g., policies and training), physical security measures (e.g., locked offices and security officers), and technological security measures (e.g., encryption, firewalls, and multi-factor authentication).

HOW DO WE PROMOTE ACCURACY OF YOUR PERSONAL INFORMATION?

We take reasonable steps to ensure your personal information is as accurate and up-to-date as is required for the purpose of collection. Please note that for certain personal information, such as your current contact information, we must rely on you to provide us with the update.

HOW CAN YOU ACCESS OR CORRECT YOUR PERSONAL INFORMATION?

​WORKSAFE BC PRIVACY POLICIES:

​Our staff and shareholders work to uphold the following WorkSafe BC policies for the protection of personal information.

  1. We prohibit the collection, access, use, and disclosure of personal information except for the purpose of delivery of services and only by and to those persons necessary to deliver the services.

  2. Ou privacy policies include administrative, technical, and physical controls to protect personal information/personal data from unauthorized collection, access, use, disclosure, destruction, and disposition.

  3. We enact encryption of personal information in transit and at rest using industry-accepted standards, strong encryption techniques, and current security protocols.

  4. We commit to the secure destruction of any personal information/personal data with an option for the return of all personal information at WorkSafeBC’s request.

  5. We further require all personally identifiable information to be removed or destroyed from metadata generated by an electronic system that describes an individual’s interaction with the system or prohibits any subsequent use and disclosure of the metadata in individually identifiable form, including but not limited to sharing the metadata with third parties.

  6. Peninsula Hearing will issue prompt notification to WorkSafeBC in the case of a security breach.​
